(DIRECT PRIMARY CARE)

WHAT IS DIRECT PRIMARY CARE MEMBERSHIP (DPC):

 

A yearly or quarterly membership fee is paid

 

In exchange patients have access to unlimited patient visits and quarterly blood labs

BENEFITS:

 

Unlimited patient visits (Functional or Chiropractic)

Unlimited Telehealth visits

Standard blood labs included (CBC,CMP,Urinalysis) quarterly

*Hundreds of other labs

*Medicines not included

Quarterly Membership:

OVERVIEW:

​

Only Yearly and Quarterly memberships are available

Cards will be kept on file and will automatically deduct membership fees annually or quarterly

No other charges (apart from medicine) will be charged to patients

All adults must undergo a New Patient Visit (separate) before they are eligible-Children are exempt

Can cancel Quarterly after 12months

Cancelation fees for No Show or for under 24 hr cancelation are $50

Canceling before 12 month cycle would be a $300 penalty

  $200 Credit for successful referrals to businesses
